Clinical Pharmacist/Specialist

Oncology Ambulatory

Job Summary:

The Oncology Pharmacist is responsible for all chemotherapy medication services to the Hematology/Oncology patients. The clinical pharmacist/specialist is responsible for assistance in order origination, order verification, clinically referencing lab values for appropriate dosing of chemotherapy and non-chemotherapy drugs for standard of care therapies, as well as investigational therapies, communication and therapy coordination for oncology patients, including being a liaison and educator to the oncology staffing pharmacists, patients and providers. Clinical pharmacists/specialists will be committee members for various multidisciplinary workgroups. Additionally, the clinical pharmacist will precept P4 students, PGY-1 residents and PGY-2 residents. This position will work under a Collaborative Practice Agreement in an ambulatory setting, with minimal inpatient weekend and holiday coverage. The practice site setting is an off-campus clinic with multi-focused coverage of both hematologic and oncologic cancers, along with some benign disease coverage.

Position Shift:

  • Schedule (follows the hours of the clinic)
  • Rotating On Call
  • Rotating weekend/holiday coverage

Qualifications:

Required:

  • PharmD
  • Pharmacist licensure in the state of Tennesse (or eligible)
  • Completion of an ASHP-accredited PGY1 residency or equivalent

  • Board Certification in Oncology (BCOP) (when eligible)

Preferred:

  • PGY2 training in Oncology
  • Experience working in an academic medical center

Department Summary:

To provide outstanding patient care by optimizing medication use and ensuring patient safety. Our commitment extends to our patients, customers, & community and encompasses service, teaching and research to optimize medication-related outcomes.

Key Responsibilities:

  • Reviews drug orders and/or prescriptions to ensure the safe, appropriate and cost-effective use of medications. Prepares and dispenses medications, provides drug information and manages medication therapy in accordance with federal, state and local regulations and hospital policy.
  • Provides outstanding patient care by optimizing medication preparation and resolving medication-related problems.
  • Maintains the quality, safety, and consistency of clinical Pharmacy services in assigned areas.
  • Supervises the activities of assigned support personnel and verifying the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or dispensing to healthcare providers for administration to patients.
