Role: Ensures the provision of optimal, cost-effective antimicrobial pharmacotherapy for individuals and populations in collaboration with a growing antimicrobial stewardship program. Provides leadership and support of antimicrobial drug-related quality improvements and research. Also provides experiential education for present and future health care practitioners. These services are provided in compliance with federal and state regulations; Joint Commission (TJC) standards; VUMC best practices, and hospital policies.
Is a leader and mentor of antimicrobial management and clinical programs
Recognized leadership in infectious diseases and antimicrobial stewardship
Creates national visibility by presenting clinical information/programs such as poster presentations, oral abstract presentations, or publishing in nationally recognized journals
Maintains high level of knowledge of current standard of practice for antimicrobial stewardship via medical literature and conferences
Ensure pharmacy staff competency with the evaluation of patients to provide antimicrobial stewardship management
Track antimicrobial usage and identify processes to maximize patient safety while minimizing medication cost
Lead antibiotic reporting to NHSN
Actively participates in statewide antimicrobial stewardship initiatives and activities
Collaborate with experts in ID, microbiology, quality/safety/risk prevention, pharmacy and others to direct reporting requirements for antimicrobial utilization and resistance.
Work collaboratively with team members, pharmacists, pharmacy technicians, clinical pharmacy specialists, and other health care team members to expand antimicrobial stewardship efforts and implement new programs.
Provide routine monitoring of antimicrobial stewardship dashboards in collaboration with pharmacy and ID physician leadership.
Lead antimicrobial stewardship compliance efforts for TJC and other regulatory and quality standards and best practices for antimicrobial stewardship.
Rounds with ID consult service and evaluates patient pharmacotherapy for appropriateness of drug selection, dosage, dosage form, route, and duration, taking into account individual patient characteristics and clinical status.
Maintain collaborative, positive interactive relationships with other members of the health care team in discussion of patient pharmacotherapy and disease management issues.
Support Pharmacy, Therapeutics & Diagnostics Committee activities including clinical initiatives, formulary decisions, medication use evaluations, adverse event monitoring and reporting, and medication policy development through the Antimicrobial Subcommittee.
Triage non-formulary requests for antimicrobial therapies and manage the antimicrobial drug formulary.
Collaborate with Evidence-based medicine staff on order sets development and review.
Leads and develops treatment guidelines, protocols, standard operating procedures and policies related to antimicrobial therapy.
Identify and document antimicrobial stewardship and other interventions in eStar electronic medical record.
Report ADEs in VUMC's confidential occupational occurrence reporting system and collaborates with departmental and medication safety leadership to ensure continuous improvements in antimicrobial medication safety.
Provide health care team members with accurate responses to drug information questions in a timely manner utilizing available primary and tertiary reference electronic and written information sources.
Leads and participates in collaborative research projects.
Provide educational programs to pharmacy, nursing, medical staff and other related disciplines on antimicrobial stewardship practices and clinical initiatives.
Precepting of Pharmacy residents and students.
Demonstrate a service orientation toward patients, staff, and visitors.
Complete all required professional and department competency assessment programs as required.
