Job Summary

Serves in expanded nursing role to coordinate referrals and coordinate care of patients. Assesses individual patient needs. Collaborates to plan the flow of appointments and diagnostic testing. Provides patient education throughout the continuum of care. Assists with coordinating multidisciplinary evaluation and followup of these patients and their families.

  • Triages all new patient referrals.
  • Assesses need for diagnostic studies prior to physician office visit.
  • Serves as initial contact to patient explaining process for visit including completion of patient history form, ensures timely completion of form before scheduled appointment and upon receipt of patient history form, enters data into appropriate computer application.
  • Requests outside records, films, pathology slides and submit for review prior to clinic appointment and monitor progress of information requests and reviews.
  • Coordinates referrals to specialty departments for multidisciplinary care.
  • Arranges referrals and transfer of records to outside institutions as needed for specialty consultation and follow up.
  • Triages phone calls of follow-up patients with questions and new complaints to assess needs and resolve problems.
  • Refers patients to community resources as indicated.
  • Monitors patient no show and cancel lists to ensure patients are not lost to follow up and documents changes as appropriate.
  • Monitors pending list and assists with scheduling as needed to maintain patient follow up.
  • Active participant in all initiatives including care coordination and data collection responsibilities.
  • Educates patients and families regarding scheduled appointments and studies.
  • Provides information and support by telephone for patients and families if further questions or explanations are needed.
  • Assists with all research efforts including data collection.

Education

Bachelor's Degree-Nursing (Required)

Experience

Minimum of 5 years-Clinical (Required)

Certification(s) and License(s)

Basic Life Support Certification - American Heart Association (AHA); Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania
