Reporting to the Coding Manager, the Medical Records Coder is responsible for coding; analyzing and abstracting patient records both retrospectively and concurrently in accordance with UHDDS, Medicare, MediCal and Tenet coding guidelines using ICD-10-CM, CPT.Coder will review medical records to assign diagnosis and procedure codes, creating APC, MS-DRG, or APR group assignments.Communicates with the physician and/or hospital departments for clarification of documentation/registration/charge issues.Reviews medical records to correctly abstract POA indicators, discharge disposition, physician consultations, etc.Keeps abreast of coding guidelines and reimbursement reporting requirements. Reports concerns to assigned Coding Quality Coordinator and/or Manager for resolution.
College level course work including medical terminology, human anatomy & physiology, pathology.
Successful completion of AHIMA or other credentialed coding program.
One to two years minimum acute care hospital coding experience including surgical coding required.
Excellent computer, communication & interpersonal skills and ability to work independently.
